INSURANCE AND COSTS
NEW POLICY - NO INSURANCE BILLING TO TAKE PLACE AFTER APRIL 1, 2008. The holistic nature of our medical practice requires patients sign a waiver acknowledging that natural/holistic care is not a covered benefit and as such must pay for all.
Robert Levine, M.D. is now participating with and accepting some insurance plans (applies only to covered services). The health plans that we are participating with are as follows:
Blue Cross Blue Shield Traditional Plan
Blue Cross Blue Shield PPO
Blue Preferred Plus
Blue Cross Community Blue
some private insurers

We are not partipating with any managed care or HMO plans.
Please be advised that health insurance plans, including Blue Cross, only pay for covered medical services. This means that certain services available at Elle Medical are not covered and may be the responsibility of the patient. Our facility specifically requires patients to review and sign an acknowledement and waiver in the event you elect to receive these non-covered services. Patients are expected to provide a credit card and an authorization which guarantees payment for copays, deductibles, administrative fees and non-covered services in the event that claims for non-covered services are denied. Examples of services that may not be covered are as follows:
· Complimentary and/or Alternative Medical Evaluation and/or Treatment
· Acupuncture Therapy
· Bio-Identical Natural Hormone Therapy
· Natural Hormone Therapy (Armour Thyroid)
· Vitamin Therapy
· VIP Executive Preventative Evaluation and Physical Examination*
· Nutritional Therapy or Counseling
· Laser Treatments or Photo Therapy
· Guided Imagery, Relaxation or Mind/Body Therapy
· Massage Therapy or Spa Treatments

Please note that all of the above services are covered if your physician makes a referral to us for a specific issue (refer to Consultation Request Form). This applies only to health plans we participate with.
We are pleased to serve our patients as their primary physician or for specialty care as deemd appropriate, however this practice is primarily consultive in nature due to the intensity and complexity of our patients' medical needs.
Please note that a referral from your primary doctor or your specialist will ensure that your consultation visit will be covered and billable (regardless of the service provided). A printable copy of the referral form is available below:
